From the people who answer our phones to the people who come to your home, we have assembled a team of professionals with big hearts. Working together we each bring our knowledge, insight and wisdom to guide you through the aging and elder care journey.

Our team includes:

Sharon Dickol, BS, CMC, AgeWise Solutions’ owner, and Geriatric Care Manager, has more than 30 years of experience in aging services, program and policy development, and services administration at the local and state level. Sharon completed her undergraduate work at the College of New Jersey and earned her GCM certificate at the University of Florida in Gainesville. She is a graduate of the Partners in Policymaking advocacy training program (1995) and the National Association of Area Agencies on Aging Leadership Institute (2008). She has presented at local, state, and national conferences on topics related to aging and caregiver interventions since 2001.

Sharon is an Advanced Professional member of the Aging Life Care Association, an organization that is nationally recognized for professional standards and ethics in care management for older adults. Additionally, she is certified by the National Academy of Certified Care Managers (NACCM), the recognized credentialing organization which assures individual competence to perform the full range of care management tasks.

 

Celeste Galati, MSW, received her Master of Social Work degree and a certificate in Gerontology from the Catholic University of America in Washington, D.C.  She worked as a social worker in the DC metro area for many years in a wide variety of service settings.  She went on to establish one of the first private geriatric care management firms in the country in 1986.  Eventually, Celeste moved to Delaware and focused on raising her family.  Never having lost her passion for working with older adults, Celeste was an eager and welcomed addition to AgeWise Solutions in March 2015.

Celeste brings a quiet warmth to her role, helping clients and families carefully address the complexities they face.  She is able to focus on long-term needs while staying tuned into the small details that positively impact one’s quality of life.

 

Sharon Custer, RN is a registered nurse with comprehensive experience in geriatric care and health management.  She began her professional nursing career in 1978 in medical/surgical and protective care nursing.  In 1996 she fell in love with geriatric nursing and has focused her career on the care of the elderly ever since.  She has both professional and personal experience caring for aging adults.

Sharon has held numerous roles in a wide variety of settings, including skilled rehab,  Assisted Living,  Adult Day Care,  and Home Health Care.  She has extensive knowledge in Alzheimer’s Disease/dementia care and end-of-life care.  Sharon is known for her compassion, dedication, and communication skills when working with the elderly.   She is a firm believer in the preservation of dignity as people age.

 

Lisa Kane, MSW, LCSW, earned her Master’s Degree in Social Policy and Practice from the University of Pennsylvania. She has been able to effectively apply her skills and qualifications within school, community, and clinical settings. Lisa has extensive experience and skills in crisis intervention and counseling techniques.  She is a member of the Aging Life Care Association.

Lisa has experience working in clinical settings as a Social Worker at Bryn Mawr Rehabilitation Hospital and at St. Lawrence Rehabilitation Center where she served as a Stroke Program Manager. She was responsible for psycho-social evaluation, coordinating therapeutic services, discharge planning, and insurance coverage. She particularly enjoyed providing support to her patients and their families during and after hospitalization.

Lisa takes pride in her volunteer community service roles. She has served on the Kutz Home Board since 2013. She has also served on Congregation Beth Emeth’s board as Social Action Coordinator and Executive Committee Secretary. She brings a broad range of professional and personal experience working with youth and adults of all ages.

 

Richard Hynes has been a reliable and trusted provider of adult day care and personal financial assistance to clients for more than 10 years. He assists those who need help with their day-to-day monetary affairs, document management, bill-paying, and medical claims filing.  His services are well-suited to seniors, those with disabilities, busy professionals, and anyone overwhelmed with mail and unpaid bills. Rick will serve as Financial Power of Attorney in cases where banking and bill-paying support is needed. Rick is an active volunteer in northern Delaware and southeast Pennsylvania, assisting seniors and persons with disabilities obtain prescription coverage, choose healthcare plans and answer questions about Medicare benefits. He is also knowledgeable of the Medicaid application process, assisting those who are running out of resources and require long term care.
